JOB TITLE: Case Manager/Counselor




This position works directly with program participants to develop social and psychological treatment plans aimed at cultivating new interests, self-confidence and interpersonal relationship skills to assist in the recovery process. This position is expected to perform a number of tasks including reviewing referrals and developing treatment plans, individual and group psycho-educational counseling, case management, and facilitating linkages to treatment and other related services. This position requires an ability to maintain strict confidentiality and handle information with tact and diplomacy.

TASKS (other duties may be assigned):

  1. Reviews referrals to determine appropriate course of action
  2. Develops & conducts individualized treatment plans with measurable goals and objectives;
  3. Develops & conducts comprehensive discharge plans
  4. Prepare client progress reports as required by DBHS and Medicaid guidelines
  5. Provides individual and group psycho-educational counseling for clients;
  6. Coordinate, prepare and maintain all required documentation in a timely and accurate manner;
  7. Participate in case conferences and treatment team reviews of client progress
  8. Develops community resources to support individuals in attaining services such as housing, additional mental health care, psychiatric medication, medical resources, financial assistance, legal advocacy, etc.; empowers clients by educating them on how to seek and utilize these services
  9. Participates in debrief sessions, weekly supervision, agency wide and other meetings, training and development opportunities as determined appropriate by supervisor;
  10. Transportation of clients if necessary
  11. Adhere to all client confidentiality requirements and standards



  • Effective verbal and written communication skills;

  • Strong and timely documentation and assessment skills
  • Must successfully pass FBI /local and Child Protective Service background investigation
  • Must successfully pass Tuberculosis screening
  • First Aid/ CPR certification; Behavior Management training

Minimum of a Bachelor’s degree in human services or related field and one year experience providing mental health and substance abuse services; or Bachelor’s degree in non-related field with four years’ experience providing mental health services; or meets the qualifications of a Qualified Mental Health Professional- Eligible; or meets the qualifications of a Qualified Paraprofessional in Mental Health
