JOB TITLE: Case Manager – MHSS

PERFORMANCE LEVEL: Direct Care

IMMEDIATE SUPERVISOR: Clinician

Description:

This position works directly with program participants in a structured environment or community setting to develop social and psychological treatment plans aimed at cultivating new interests, self-confidence and interpersonal relationship skills. This position is expected to perform a number of tasks including assessing client needs and providing access to the appropriate services including treatment, rehabilitation and other related service linkages; monitors cases for progress and acts as an advocate for the client.

Essential Duties & Responsibilities:

  • Reviews referrals to determine appropriate course of action
  • Develops & implements individualized treatment plans with measurable goals and objectives;
  • Develops & implements comprehensive discharge plans
  • Prepares client progress reports as required by DMAS and licensing guidelines
  • Provides individual and group psycho-educational support for clients;
  • Coordinates, prepares and maintains all required documentation in a timely and accurate manner;
  • Participates in case conferences and treatment team reviews of client progress
  • Develops community resources to support individuals in attaining services such as housing, additional mental health care, psychiatric medication, medical resources, financial assistance, legal advocacy, etc.; empowers clients by educating them on how to seek and utilize these services
  • Participates in debrief sessions, weekly supervision, agency wide and other meetings, training and development opportunities as determined appropriate by supervisor;
  • Transportation of clients if necessary
  • Adheres to all client confidentiality requirements and standards
  • Other duties as assigned

Minumum Requirements:

Meets the qualifications of a Qualified Mental Health Professional Adult (QMHP-A), Qualified Mental Health Professional-E (QMHP-E),  or meets the qualifications of a Qualified Paraprofessional in Mental Health (QPPMH).

Knowledge, Skills & Abilities:

  • Demonstrated ability to develop, monitor, and evaluate treatment plans
  • Ability to assist clients using therapeutic interventions to assist in the development of mental, emotional, life and social skills
  • Knowledge of DBHDS, Medicaid and AYFS client treatment standards
  • Knowledge of area serviced and community
  • Demonstrated cultural sensitivity pertinent to the clients and the communities served
  • Ability to travel throughout the community to make home visits if necessary, attend meetings and participate with community educational programs and services
  • Ability to communicate effectively both verbally and in writing; completes documentation according to DBHDS, Medicaid, and AYFS guidelines
  • Strong team/consensus building skills
  • Ability to effectively resolve conflict and cope with crisis situations
  • Strong and timely documentation and assessment skills

Conditions of Employment:

  • This is a part-time with the possibility of full-time non-exempt position
  • Satisfactory completion of Background Investigation and Child Protective Services check
  • Valid driver’s license and proof of vehicle insurance coverage if transporting clients
  • This position requires on-going AYFS Supervision
  • CPR/First Aid Certification/Behavior Management Technique training
  • Accessible by phone 24 hours a day/7 days a week/365 days per year
  • Negative TB Screening

Physical Demands:

While performing the duties of this job, the employee is regularly required to drive, sit, talk, hear, stand and walk.  In addition, the employee may be required to restrain a client.
